Coordinates access to health-related services by assisting clients with utilizing available resources.
Identifies potential clients, completes initial needs assessment, and maintains follow-up to ensure appropriate guidance to health services.
Provides informal counseling, education, and social support to clients. Assists clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.
Develops and maintains strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members).
Maintains accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.
Assists members with member-specific needs, in-person, virtually and by phone, including health coaching, assistance with application for public aid, payer specific programs, medication access, and/or disability benefits.
Transports donated items, food boxes, and other resources to members.
Performs home visits for non-medication-related concerns (e.g. safety checks or social visits).
Performs non-clinical member assessments as needed.
BLS Provider obtained prior to hire date or job transfer date.
Drivers License obtained prior to hire date or job transfer date.
Medical Certification or Associate’s degree in healthcare, social work, wellness or related field required. Bachelor’s degree in healthcare, social work, wellness or related field preferred
Two (2) years equivalent work experience in a field related to healthcare, wellness or similar required.
