CentraCare Health – Monticello Care Center is an 89 bed skilled nursing facility located on the banks of the Mississippi River. The care center provides 24-hour nursing care to long-term care residents and rehab residents.The RN/LPN, under general supervision, plans, directs and evaluates total nursing care of residents; relates effectively with other shifts for continuity of care; participates in education, maintenance of records and upgrading of policies, procedures and skills, all in accordance with applicable standards, guidelines and regulations. Responsibilities include, but are not limited to:· Participates in medication pass audit process as part of process improvement plan to reduce medication errors· Participates in and promotes regulatory compliance and survey participation· Monitors stations for regulatory compliance and proper documentation and takes corrective action when needed· Performs daily rounds to observe assigned residents to determine if nursing needs are being met· Works with area/service staff in problem-solving· Recommends programs, services, processes, and systems based on a needs assessment and on the Strategic Plan· Recommends goals, objectives and strategies for service/area programs and services that support organization's Mission, Vision, Strategic Focus and budget· Monitors and makes adjustments based on work requirement in day-to-day staffing and scheduling in area/service of responsibility· Performs nursing duties, including administration of intravenous fluids, medications, and therapeutics based on resident's individual plan of care· Supervises the delivery of care by licensed practical nurses and nursing assistants· Documents in medical records the resident care provided, the resident's response to treatment, and develops appropriate plan of care for resident.· Completes admission assessments per protocol. Other duties as described in full job description· Minimum Qualifications:· Current MN RN or LPN license· Previous RN/LPN experience· Current BLS certification· Geriatric nursing experience preferred· Basic computer skills· Ability to handle multiple tasks simultaneously· Organizational, problem solving and decision making skills· Ability to work in a team environment and/or work independently as needed· Ability to read and communicate effectively in English· Strong written and verbal communication skills· Strong interpersonal human relations skills
