Social Worker I: The Social Worker I (LSW) provides, coordinates and administers social services for patients and residents in accordance with applicable standards, guidelines and regulations. Complete psychosocial and admission/initial assessments, case development, problem solving, discharge planning activities and other services that are complex in nature, and/or involve support or referrals, that can best be provided by a social worker. Services to be provided may include, but are not limited to: support with initial nursing home placements, home care and hospice referrals, domestic violence resources, support to patients/residents and staff in situations of suspected maltreatment cases, coordination for mental health services, grief and loss support and resources, support, education and advocacy to patients/residents and other referrals to community resources as appropriate.Requirements: · Bachelor's degree in social work.· Current Minnesota licensure in social work (LSW, LGSW or LISW).· Two years experience in healthcare.· Basic computer knowledge· Knowledge of Joint Commission, MN Department of Health standards and various other regulatory standards. LANGUAGE SKILLS: Ability to read and communicate effectively in English.Strong written and verbal skills.Strong interpersonal human relations skills.
