*PATIENT CENTERED CARE FOR THE VETERAN: To deliver whole health, the Hudson Valley VA is applying the Elements of Patient Centered Care - a model that starts with an exploration of what matters most to Veterans and partners with them to achieve their best possible health. *Personalized:A personalized approach requires a dynamic customization of self-care and professional health care strategies. These must be specifically relevant to the individual, based on factors such as their medical conditions, genome, needs, values, and circumstances.

Proactive: A proactive approach uses strategies that strengthen each patient's innate capacity for health and healing, such as mind-body therapies and nutritional changes to build resilience prior to surgery or chemotherapy.

Patient-driven: A patient-driven model is rooted in and guided by what matters most to the person, aligning their health care with their day-to-day goals and lifelong aspirations. This is the most critical component, because a personalized, proactive approach that is not based on the individual's values will have little impact. A person who is engaged and inspired is the most likely to achieve the highest possible level of health and well-being.**
