The Women’s Hospital at Henrico Doctors’ Hospital offers a full range of women’s health services. Central to the management of the care of our patients is the assessment of the psychosocial aspects of the patient and family to identify high risk psychosocial indicators, such as maternal substance use, domestic abuse, single parenthood, lack of financial resources, housing needs, teen pregnancy, family crisis, and bereavement. These at risk factors need to be screened for and managed by a professional social worker along the continuum of the hospital stay and in tandem with the multidisciplinary team.
SPECIFIC ELEMENTS AND ESSENTIAL FUNCTIONS*
*1. Social worker will have knowledge of and practice according to the guidelines established by the National Association of Social Workers (NASW).
*2. Develops and maintains an understanding of the history, traditions, values and family systems of patient groups as they relate to health care and decision making.
*3. Maintains appropriate safeguards for the privacy and confidentiality of patient information.
*4. Acquires and maintains current knowledge pertinent to social work practice. Social worker shall demonstrate a working knowledge of current theory and practice and integrate such information into practice. Social worker shall maintain education requirements to keep license current as a Licensed Clinical Social Worker (LCSW).
*5. Analyzes pertinent data, communicates findings and documents information. Provides ongoing assessment, including gathering comprehensive information to use in developing interventions and treatment strategies.
*6. Initiates and documents patient teaching and discharge planning including family and/or significant other in collaboration with multidisciplinary team members. Social worker implements intervention and treatment plans that promote patient well-being and ensure a continuum of care. Planning shall be based on comprehensive, culturally competent assessment with interdisciplinary input.
*7. Advocates for the needs and interests of patients and patient systems including advocating for larger system change to improve access to care and improved delivery of service.
*8. Acts as an educator for patients, families, the community and other professionals regarding impact of illness and disease progression, advocacy for benefits, and adherence to treatment regimens.
*9. Communicates effectively with peers, supervisors, ancillary department and other customers. Social worker shall participate in care teams, and collaborate with other professionals, volunteers, and groups in and outside of the practice setting to enhance all aspects of the patient and family system’s care.
*10. Maintains documentation of social work services, which reflect the patient and patient systems’ pertinent information for assessment and treatment; social work involvement and outcomes with and for patients; and in accordance with care goals and legislative and administrative regulations and policies within unit Scope of Care.
*11. Performs other related duties as assigned.
EXPERIENCE Required - Master’s Degree in Social Work (MSW) and one year experience as a medical social worker. Preferred - Current certification by the state of Virginia as a Clinical Social Worker (LCSW). Two years experience as a medical social worker field, including one year in the perinatal setting.
EDUCATION Required - Graduate of accredited School of Social Work with MSW degree. Preferred - Completion of post graduate studies and completion of LCSW exam.
