Wisconsin State Government Behavioral Health Social Worker / Case Manager in Waupaca, Wisconsin
Waupaca County Behavioral Health is looking for a trauma-informed, strength based, and person centered Social Worker or Case Manager to provide services to chronically mentally ill individuals and/or those experiencing mental health crises. This position will provide crisis intervention, de-escalation, and services to individuals who are currently, or recently have, experienced a psychiatric crisis. What to Expect:

Provide face to face mobile health crisis coverage.
Part of an on-call rotation (nights and weekends). Position Summary: Responsible to provide mobile emergency mental health crisis coverage, which includes assessment, intervention, de-escalation, coordination, and referral on an on-call rotation including nights and weekends. Responsible for conducting intake activities and screens as assigned to provide a crisis plan that outline current personal issues and what services are needed to provide support and safety in a community setting. Provide case management services to help eliminate current gaps in treatment that will assist the client in being more successful in treatment compliance and avoiding the need for re-hospitalization. Transport clients to appointments, interviews, tours of service locations etc. Meets regularly with community services providers to coordinate and monitor client progress; this could include psychiatrist, psychotherapist, employers, crisis facilities, legal system, AODA counselors, day service providers, and housing authorities. Attend staff meetings and trainings as directed.
EDUCATION: Graduation from an accredited college with a degree in social work and/or human service field required. Wisconsin Social Worker certificate required to be classified as a Behavioral Health Social Worker. Social Worker certificate not required to be classified as a Behavioral Health Case Manager.
