Job Summary:

Under limited supervision, responsible for assembling and maintaining patients’ health information in medical records and charts.

Essential Duties:

  • Assemble patients’ health information in chart.

  • Ensure all forms are accurately completed, properly identified, and signed.

  • Ensure all necessary patient information is in the computer and up to date.

  • Scan pertinent information into patient’s EMR.

  • Obtain and handle medical record document requests from outside parties, which may include subpoenas, FMLA forms, etc. for response and regulation compliance.

  • Perform other duties as assigned.


  • High school diploma or equivalent required.

  • Completion of medical record technology or medical terminology course preferred.
