Fairview Health Services Physical Therapist Assistant - Homecare and Hospice in Minneapolis, Minnesota
Fairview Senior Services is looking for a Physical Therapist Assistant who will provide direct patient care in Ebenezer assisted living, independent living and memory care settings in the west metro. Locations included are Minnetonka, Plymouth, Deephaven, Orono, St Louis Park. PTA may be asked on occasion to help teams in other locations in the metro area as well. The Physical Therapist Assistant will be supporting 2 Physical Therapists who currently see patients in these locations.
The Physical Therapy Assistant is responsible for providing direct patient care in the home or long-term care setting based on treatment plan established by primary Physical Therapist.
64 hours in two weeks, M-F, Day shifts

Territory: Main base will be in Minnetonka, Plymouth, Deephaven, Orono, St Louis Park.
The phyical Therapy Assistant is responsible for providing direct patient care in the home setting based on treatment plan established by primary Physical Therapist.
Implement patient treatment plan as instructed by physical therapist
Operates therapeutic equipment and devices as indicated in the plan of care
Documents patient care delivered, physician orders and other pertinent patient information in the medical record within established guidlines
Communicates any patient care issues or changes in patient status to the Physical Therapist and/or patient case manager
Participates in educational programs and agency committees as appropriate. Accepts responsibility for the understanding and implementation of the applicable content of all departmental staff meetings.
Demonstrates ability to provide care or service adjusting approaches to refect developmental level and cultural differences in the population served
Licensed as a Physical Therapist Assistant and eligible to practice in Minnesota
Current BLS (AHA or American Red Cross) for health care providers certification
1 year recent clinical (work) experience as a Physical Therapist Assistant
Valid Minnesota drivers license (or license that matches your current permanent residence).
Current and adequate automobile insurance policy
Computer skills: able to effectively document in EMR
Previous home care experience
Experience working the geriatric patient population and dementia care.
