University of Minnesota Physicians RN Care Coordinator (Cardiology - Congenital Genetics) - 1.0 FTE in Minneapolis, Minnesota

As the multi-specialty group practice for the faculty of the University of Minnesota Medical School, University of Minnesota Physicians has more than 950 physicians and 1,600 health professionals, providing clinical care in over 100 specialty and sub-specialty areas. The Care Coordinator - RN proactive patient and family centered care. Facilitates a seamless patient experience between the care sites – hospital/clinic/home. The Care Coordinator works to maximize the patient’s health outcomes and the patient experience by identifying patient and family needs. Proactively manages care issues to prevent unnecessary hospital admissions/readmissions/Emergency Room visits. This includes but is not exclusive to - supporting the patients by providing patient education, coordination of care across other specialties as well as between care settings. Works to develop and instill a sense of trust and patient advocacy to be a main point of contact for patient and family regarding health concerns.

In order to qualify for this role, you must possess:

  • Current MN RN license

  • BSN

  • Knowledge of specialty area

  • 3+ years of clinical nursing experience, preferably within family medicine.

  • Proven ability to function at highest level within scope of practice

  • Patient education and pro-active disease management experience

  • Excellent assessment and care coordination skills

  • Experience with an EMR

  • Keyboarding/computer skills

  • Strong customer focus to accommodate patients


  • MSN

  • BLS Certification

  • Specialty certification or training

  • Knowledge of case management

  • Patient advocacy experience

  • Previous inpatient care experience

  • Previous experience providing patient and family centered care and care coordination across the healthcare continuum

  • Excellent time management, prioritization, multi-tasking and organizational skills

  • Ability to work in multiple computer systems

  • Skilled in using EPIC

  • Effective coaching skills

  • Critical thinking skills

  • Ability to provide lifestyle and chronic disease/complex case management services

  • Ability to cross cover for peers as needed

  • Ability to build positive relationships

  • Ability to delegate appropriate tasks while functioning within a team based model

  • Flexibility, adaptability and teamwork

  • Motivational interviewing skills
