LCMC Health Home Health RN Patient Care Coordinator in New Orleans, Louisiana
Oversees delivery of home care nursing services. Responsible for ongoing quality, productivity, efficiency and compliance with the regulatory requirements of managing a home care patient.
3 years of nursing experience required and 3+ years in home care preferred. Must provide supervision and guidance to nursing staff caring for home health patients. Job duties include review of clinical documentation, prepare OASIS admission data for transmission within 7 days of the start of care, assure interdisciplinary communication and Home Health Aide supervision takes place as appropriate, monitor documentation of compliance with standards of practice and Medicare Conditions of Participation, document appropriate ICD-10 codes on evaluation documentation, monitor clinician compliance with physician communications and 60 day summaries, perform medication review on therapy only cases and monitor medication reconciliation on nursing cases, participate in supervisory on call rotation, and participate in Emergency Preparedness activities.
NOTE: This position is considered essential personnel in the event of an emergency. Must maintain automobile liability insurance at the 100/300/100 level.
Current Louisiana RN licensure is required on hire. CPR certification is required to be current by end of general hospital/nursing orientation. Other Required certifications must be completed as follows:
Experienced RNs who transfer or are newly hired will have current department specific certification(s) by 90 days following transfer/hire.
