The RN Complex Care Manager III is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of the case management plan across the continuum of care for the member, family or legal representative.

Utilizing clinical knowledge and critical thinking skills, the RN Complex Case Manager applies case management principles to guide the member along the continuum of care to the optimal functional level and quality of life. The Complex Case Manger should utilize available resources to achieve the most favorable clinical and cost outcomes.

The Complex Case Manager demonstrates member advocacy and collaborative approach throughout the case management process. The Complex Case Manager achieves established production and quality metrics, and oversees a team to ensure quality and production metrics are met. The Complex Manager identifies opportunities for quality improvement and cost savings, and complies with regulatory and accreditation requirements.

The ideal candidate for the RN Complex Case Manager position must have critical care experience with excellent interpersonal, verbal and written communication, and organizational skills that also maintains professionalism and objectivity in member and family interactions.

  • 5 years of work experience

  • Registered Nurse (RN)

  • Bachelors or Equivalent Experience

  • CCM Certification or equivalent is preferred
